Dr. Lennox and his staff are committed to respecting your privacy and protecting your personal information and medical information. In general, personal information includes, but is not limited to, your name, age, residential address and phone number, and email address and identification number. Medical information includes, but is not limited to, the medical information contained in your chart.

Dr. Lennox and his staff are committed to protecting your privacy whether you are a patient, a prospective patient, and or browsing our Web site. You have the right to know the types of personal information collected by us, as well as how and why it is used. It is also your right to choose whether or not you wish to provide us with this personal information.

Please read this Privacy Policy carefully so that you may understand how your personal information is managed. Dr. Lennox and/ or the office Privacy Officer will notify you of any significant changes to our privacy policy.

What Information Does our Medical Practice Collect?

We record personal information such as your name, contact information, and date of birth, and medical history. The personal information collected will only be used for the purposes originally stated, unless additional consent is obtained.

How do we use the information that is collected?

Dr. Lennox and his staff recognize the importance of being able to contact our patients in a timely fashion, using a variety of methods, including email if provided by our patients. All personal contact information remains confidential.

Disclosure to third parties.

Dr. Lennox’s office will NOT release information to third parties without prior consent. Like other medical practices, Dr. Lennox may use external providers for certain medical diagnostic, medical therapeutic, administrative or financial duties that may require access to personal information. Finally, we may release personal information when required to do so by law.

Technological measures include password protected access for all users in the office and software programs that detect intrusion and viruses. Dr. Lennox uses a sophisticated Electronic Medical Record that is externally backed up and has the highest level of approval from the BCMA Physician Information Technology Office.

How can you update your personal information at our medical office?

Our office makes every reasonable effort to keep your information accurate and up to date. You can help by keeping us informed of any updates such as address changes or change of name. If you find errors in your information, let us know immediately and we will make the appropriate corrections.
